What is the address I cannot find it on Socialsecurity.gov, can you please email me the information where I fill out the form for extra help, I cannot afford a $800 a month copay for my drugs each month or to be denied. I filled out the form over the telephone at the pharmacy while I was having a panic attack, dizzy and felt like I was going to have a heart attack after 2 days of no meds. I called in my new address but could not change it on the form, I would have liked to review the form to insure I will get extra help I need but the women I talked to said I could'nt.
